Membership Questionnaire
We ask anyone interested in pursuing membership at The Chapel to complete a Membership Questionnaire. This form gives you the opportunity to let us know what you believe and how The Chapel can better minister to your needs. It also reaffirms your commitment to support The Chapel’s doctrinal beliefs and ministry endeavors. Upon completing the questionnaire, return it to the Assimilation Ministry. An elder will contact you to arrange for an interview. These brief interviews are usually conducted on Sunday morning or at a mutually convenient time.
